At least one of these reasons must apply if you're asking for your social work bursary to be extended:
• either you or a very close family member have a serious illness
• there's been a death in your family
• your placement started late because of a shortage of available places (but not if you previously refused a placement)
• the placement broke down and you need to start again (but not if you caused the breakdown).
To request a social work bursary extension you'll need to ask your programme leader to complete an SWB04 form for you.
